 I have to say, I've seen this referenced as having a December release, however looking at the labels, I see it is "12 9 1980" - which, in UK format, should mean September 12.

Factory, at least according to the release schedule from 1980 reprinted at factoryrecords.org, was releasing records on Fridays. December 9, 1980 was a Tuesday (John Lennon's murder), where September 12 was a Friday. I think it should be considered a September 1980 release. Neither version charted, unfortunately, so its hard to pin it down.

 I added some label scans to the NZ 12" some time ago. (I've just linked it ).
I thought my copy was UK - see my earlier comments there for why I wasn't sure.
My labels look the same as Paul's scans, I think!
(I do have the embossed cover - and no "limited edition" sticker - as on the NZ version)

 If the image in the centre of the sleeve is embossed then it is a 9 December 1980 issue (ie. first 12" issue). The 1981 re-issue had flat sleeves with no embossed images. I also think the font on the labels looks slightly bolder on the 1981 re-issue.
