Yes, this is the famed guitarist and as a not so talented vocalist his first time around according to the Review. According to the Seller (on Discogs), it was purchased from the Bell Estate about a year before he passed, one of two copies.

Rich Howard 24th Feb 2022 | | Vinyl AlbumJack Larson And Oscar Velez - The Official Request Album | I got around to getting the images taken from the recent eBay auction and cleaned them up as best I could.
This would no doubt be the same Jack Larson as on the FRATERNITY label. Sources tag a 1980 date to the release, but I can't be certain. The label (O.J.) would likely be a private issue, the letters taken from their first names, Oscar and Jack.

I have the LP, but not the script. And those other pieces seem to have come with a dummy, but not the record itself.
There was a second LP issued in 1966 that also possibly came with that same script shown here (copyright 1964). And from what I was told, there may have been at least one or two more later on.

It is legitimate and possibly preceded the other image here given the quality of the image overall when compared to the more common issue. Backs of both are identical. The image was taken from an eBay auction and squared as much as I could at the time, but at least shows everything.

Artist on the disc is shown as Vess Ossman, and it is in fact a 7" disc (my error), correction for change already input. The A and B sides are noted on the label, 174-A/ 174-B, as well as mirror-stamped in the dead wax.

Rich Howard 30th Jul 2021 | | Vinyl AlbumDel Shannon - Little Town Flirt | Given that "Runaround Sue" on this LP is supposed to be a cover of Dion's song hit, I'm trying to figure out how this LP came out before Dion's version ever surfaced. Dion's single came out in August, while his titled LP came out in October. So how could this predate them by coming out in June...?
Also there's a problem in the Notes above with this being "Del's first LP"., when his RUNAWAY album was clearly issued first.
