With this edition, the editor introduced separate Issue numbers as well as retaining the ongoing Volume/Issue numbers. Alas the maths were faulty (there having been only three editions in Volume 1) and so this "Issue 14" is really only the 13th. This error would never be corrected, even when the Volume/Issue numbering system was scrapped.
