How do we handle these? I have come across a 2 disc set in a gatefold (Hanoi Rocks), one is an 33 LP, the other a 45 Maxi. The album exists w/o the maxi, it looks unlikely the Maxi was availabble on its own. I think I do have some more of these.

If it's a bonus (ie a limited or special edition with extra tracks/remixes etc), then add the 12" on it's own, putting in Notes that it was available with an LP.
However, if it's actually part of the standard complete LP, then it probably shouldn't be included here.

Ah, no - I'd say that isn't acceptable. Those three tracks were universally released as part of the full album, including the vinyl reissue of 2000 and a recent cd reissue. It isn't in itself a bonus, just an extension of the LP release...so I'm afraid it'll have to go...
What about those 12" singles in 90s that came out as one disc in a gatefold sleeve, with space for the second disc, released a week later with a different catalogue number. Sleeve often had 'special collectors edition' or similar, saying part 1 or 2 record set. Second one usually in a simple picture sleeve that fitted inside the gatefold. the fact that the second disc was available commercially in its own right, with its own catalogue number, suggests to me they should have separate entries. What do others think?

Yes, I'd agree - if they were not purchased as one item then they should be separate, with a Note added to explain.
