 For what it's worth: I have this release also and my copy includes indeed the 16-page booklet. Labels are the black ones....

 With respect to Trainman's contribution, I've updated the images of the gatefold interior. Also added additional images of a different label variant.

 Interesting.

Coveboy's labels are a ca. 1969 or later pressing. My copy is the original black label and mine does NOT have the booklet (I'll be posting images in a few minutes).

RPFiller, what labels does yours have? I wonder if the booklet was added to later pressings AFTER the film became such a mega-hit or if it was just the first pressings with the black label
