With humble respect 2 coveboy, complete left/right gatefold scans added, with slightly more focused scan of Dr. Demento's liner notes.

The yellow labels uploaded by coveboy are 1975 or so, as they have the Warner Communications blurb on the label as well as the Rockefeller Plaza address.

This grey(?) Atco label came into use in late 1978, judging by the singles labels in 45cat.com. The earlier pressings would have had the yellow Atco label with the logo on the left-hand side.

SP on label and runout, means pressed by Specialty I believe.
The PR is probably Presswell.

 Can A: 0-4 SM 1-2.
B: 0-5 SM 1-3 mean that Columbia Records pressing plant in Santa Maria was used in one way or another?
