 The label name does not appear on this package so I looked to the Metal Archives where all versions released are on Nuclear Blast.

 Nuclear Blast is a German label, but it seems there were a lot of reissues from another source trying to pass themselves off as NB releases. Given what you've said about the insert, it's possible this is one of those.

Actually, The Windings was only a fifteen-minute walk from where I'm sitting now. Quite a few bands recorded there, including Chumbawamba (Disc Two of this) and Hecate Enthroned.

 This is Death Metal music. The original packaging was probably a slimline case. The insert is printed one side only and folded over to fit. It has been cut and chopped to fit a regular jewel case.
