Unlike volumes 1 and 2 of the series (Muddy Waters and Howlin' Wolf) this does not have a jewel case- fold out cardboard package with booklet enclosed. Also first two volumes came out in 1994; this shows a date of 1997. Looks like US Chess was using the CHD prefix through at least 1999. This has MCD,
