Was this actually a USA release with this number, or should it be combined with UK issue?
I do note the "EMI International" on the back cover here, though my copy was found in a more UK style "EMI Records Limited" sleeve.

Described in Notes as a 4 page booklet, this may describe the outer full colour cover, images of those 4 pages can be seen at the linked US release. The remaining 12 pages are black and white reproductions of old press articles and adverts, and is really interesting. I could photograph the complete thing if anyone wants to see it. Maybe later copies only included the outer cover part.
