Rappaport. This is correct, as Chunky, Novi & Ernie was a trio featuring Ilene Rappaport that featured the song on their eponymously named 1974 LP.
Rappaport went on to record as Lauren Wood achieving success as a singer-songwriter, voice over artist and commercials.

There is also seldom any way to determine if a particular generic sleeve is associated with a particular album/LP. If there is a way to determine this (ie. Catalog #), then, of course, it should be included. With the proliferation of used record stores over the past 50-60 years, many sleeves have been changed in packaging, often with no care to even the label.
