 No, two writers were involved in creating "Poor Butterfly", Raymond Hubbell (music) and John L. Golden (lyrics): https://en.wikipedia.org/wiki/Poor_Butterfly

 As there's no - between Golden and Hubell, I'm guessing that should be one name.

 All eight record labels added, apparently from an early pressing before the end of 1945, when Columbia reduced the text with the patent dates under the company name from three lines to two.

 The release day of "17 July 1944" for the album has been submitted by W.B.lbl whereas the Billboard magazine lists the album as released "from June 15 thru June 22" (see notes). We can assume though that the copy deadline of that Billboard issue was around June 15, so a possible delay could not be considered there. More probably they had forgotten what they had written in an article one week before, "a Benny Goodman Sextet album ... is to be released July 15". This article also confirms, "B. G. traveled to Columbia's plant in Bridgeport, Conn., to pick out the records he wanted in the album".

 According to the liner notes of the album, Benny Goodman personally considered these sides as his finest Columbia recordings. They were recorded between Oct 1939 and Mar 1942 by different combinations of musicians in the sextet, featuring guitarist Charlie Christian, pianists Count Basie and Fletcher Henderson, Lionel Hampton on vibraphone, Cootie Williams on trumpet, and others.
