Michael Moorcock - The Bane Of The Black Sword
If it helps, I have added the copyright page from an identical copy of this book. The $2.25 price and this catalogue number (which does appear to be from 1983) seem to have only been for the 7th printing.
In 1984 the price increased to $2.50 and there was a different cover design.
See Moorcography.org for more information.
